I'm familiar with CoC, and get what you're saying, but you might want to consider doing two modes. Sandbox, for people who are familiar with the game's mechanics and don't need any hand-holding or tutorializing, or arbitrary limitations, and then a "story mode" that guides new players through the different mechanics.
I sort of wish that CoC had an "advanced start" option that was basically a sandbox with all the features unlocked. That would've bowled me over when I was new to the game, but starting over in CoC is such a drag because of that aforementioned grind.

You're thinking about it wrong. You are supposed to put yourself into the shoes of your character - in a lot of these games if you fuck you'll either die, lose your freedom entirely or the experience is so mindblowing it ruins your life forever.
You're supposed to fight it until you, yourself, get so horny you just need to blow and walk yourself into it. It's the best way to get immersed.
Anyway Succubus Prison, Succubus Academia and Captive Alstromeria might be up your alley. SuccPri is a RPGM escape room game where you walk around a house to find items and work out what you can do at what times of day - it's a somewhat challenging puzzle but not so hard, just hard enough that it feels like an actually well thought out game.
Academia is about walking the right boys into the arms of death and trying not to die to something else on the way with some battlefucking.
Captive Alstromeria is more of what you're complaining about but the game is mostly non-stop sex that you can't get out of but you're just trying not to nut so you might be into that.
I want to assume you know about Monster Girl Quest since you didn't reference it so parts 1-3 should be your first stop if you're only just now getting into being fucked and eaten by girls.

How much do people expect from a pregnancy system, anyway? I'm just now thinking about Breeding Season and the 'pregnancy' in that is sometimes when you breed two monsters together they'd just instantly spawn a new character.
TiTS and some similar text adventures just have a 'daycare' area that all your hellspawn get thrown into like a plot black hole.
These are two complete opposites, one focuses only on the result of breeding, the other focuses on only the process. Do breed/preg fags like games like Breeding Season? Do they like how it is in TiTS?

Breeding and pregnancy are two different (even if very closely related) fetishes and which will fit into a game really depends on the gameplay and story. Games like breeding season are breeding fetish games, not pregnancy fetish games. Games with a fast-pace or action games tend to lend themself to sped-up pregnancies or pregnancies with no consequences. Slower paced games and life sims tend to lend themself to more detailed mechanics related to the pregnancy itself.
I personally prefer it when games take a slower pace to pregnancy and give detailed information instead of going straight to full term or birth but of course because of how much pregnancy can affect a character that can become very un-fun for anyone who doesn't have a pregnancy fetish but does have a breeding fetish and gets their character pregnant.

It depends on your game. If pregnancy is a means toward advancing your goals in the game, then shortening it makes sense. You have to pace the pregnancy in a way that feels rewarding to the player, but doesn't force them to wait forever for payoff they need to progress in the game. This is why breeding simulators tend to skip over pregnancies or else operate on fast time scales so the player isn't just idling in game the whole time.
But if pregnancy isn't an essential component of the game, but optional content, it makes sense to make it last a long time so players get the most out of it. For them, the pregnant woman is the point, not the breeding cycle. Some might actually care about the kids after they're born, other's not. Childcare might result from pregnancy, but not everybody's interested in simulating that.

I'm more of a secondary audience to these. I mostly just play the game and then enjoy the animations. (ever so slightly more than if I just looked up a rip on a booru or whatever because I earned it.)
But I did engage with a game in a somewhat similar way because it was an action game that's fairly challenging, and at one point I got frustrated with the difficulty and let the monsters have the character.
Virgin Invader is one game that goes the other way. You play as a demon king possessing the hero's body, and it's a bet-em-up where you defeat girls and then fuck them of summon monsters to do the same. Maybe the other MenZ Studio games are the same, I should give them a look. Anything else like that?

I think this kind of hits on the distinction between types of erotic games and how they handle fetish content in general.
Fast-paced and action-oriented games (Ayura Crisis, Eris Dysnomia, Night of Revenge) usually focus on ‘quickie’ content; get in, show some skin, and move on; fetish content is there, but largely superficial, a sentence here or scene there. Even in games with a general fetish theme like vore in Echidna Wars the game doesn’t invest much into longer content. They’ve got more in common with porn vids.
RPGs, Trainers, and LifeSims, at least the ones that are done moderately well (Degrees of Lewdity, Course of Temptation) invest more time in fetish content with an emphasis on how it affects the character in the long term; the appeal isn’t just in the sex content, but in seeing how the player characters or NPCs deal with it. These games lean heavier into corruption themes, character interactions, and what happens to the character after the sex. They’ve got more in common with erotic fiction.

For male PCs, I would appreciate games drawing attention to the pregnant status of NPCs, having the occasional scene where you get confronted about impregnating them or get asked to run their belly and feel for a kick. Keep an eye on Debasing Grounds. It has been in consistent development for a long time, with frequent updates and tangible improvements. Lilith's Throne has a better presentation but abysmal dev pace and has basically stagnated (which is a shame but, well, we did bully Innoxia out of there so we just point and laugh from afar). Broken Dreams Correctional Center is kind of like Flexible Survival, especially in terms of furry content and having to interact with furry NPCs, but in theory you could set the rest of the prison population to your preferred race/genital configurations. In FS, removing furries essentially cuts out 95% content (last time I played was years ago but I'm sure it's still true).

Debasing Grounds didn't even have art until this year. Such is the curse of customization and incomplete systems. You're more likely to find better (or at least more consistent) art with tighter narrative games, which I feel are on the other side of the spectrum from original anon's CoC/TiTS angle, though closer to ToA.
I've recently started keeping track of completed titles I actually liked playing. Headpats & Handholding (yes that's the title) had pretty good story and gameplay, if you can stomach RPGM. Glitches and Bitches (RenPy, 3D) was OK, though it felt cut short (even if the narrative was complete it seemed like the setup was for a much longer and more involved title). Aurelia was actually cut short - seems to have been snipped and pushed out, cutting out incomplete content rather than polishing things up before final. Oh and someone did RenPy remakes of Bible Black/Starless/etc, if you feel like re/playing them (check f95).

Is the artstyle consistent with the rest of the game?
Artists are incredibly flaky when you stretch them over a timespan of years. It's a very common issue that you'll have a runaway artist or two and have to get someone else.
In that scenario, you can either:

Honestly, projects should be led by artists and backed up by coders. Max the Elf for example is artist-led and has community volunteer coders, something like that. It means that even if you lose a coder and the codebase is shit ... even if it takes a year to refactor everything, you don't lose what you see on the face of the game.

Doing all art over from scratch would take longer than doing all code over from scratch unless you have a horribly overscoped game.
It's also easier to find people to do it. When it comes to games, artists are scarce but programmers are plentiful. I've not played it, but I hear Melty's Quest, by the same dev as KP, is about a girl that's pretty lose.
In Sakura Gozen, the FMC is very... horny from the start, and she even starts masturbating during the "how to play" section of the game. She doesn't start off promiscuous though, and is even a bit hesitant with men.
In Sakura Gozen's prequel, The Monstrous Horror Show, one of the main girls is extremely promiscuous and constantly making sex jokes and is often up for sex whenever. Note that these two games are very story heavy.
I've also not played Vitamin Quest, but I hear the FMC is pretty free spirited. Same for The Depravity of a Lewd Vampire.
There's honestly a good number promiscuous female lead games, but I don't know of any that are specifically:

The problem is trying to find this game as a player, and find the audience for this game as a dev. TFGS is mostly flooded by the other kind of 'sissy' game, which also means the audience (and the $) is there for it. A dev who wants $ may end up catering to the "feminize MC" crowd even when they start out from the other end. And that's on top of "sissy" being a very specific subset of "feminization" to begin with, which could mean that "general" feminization games aren't even what you're looking for.
"MC feminizes someone else" is a common enough scenario in "MC power fantasy" games as a "side chick" sort of option. If you want a game entirely dedicated to it, your best bet might be checking the "sissification" tag on f95 and reading through each thread to see what content the games offer. Off the top of my head, "The Pink Answer" might work (MC feminizes NPC one-on-one). "Sissy Maker" might seem like the perfect game, but it's a tedious grind with dubious result/goal/feedback loop that is frequently frustrating.

It's useful shorthand for things related to farming that the UK doesn't really have comparable (and certainly not as instantly recognizable) icons for. The UK has a problem that many buildings get repurposed for other things, so over time distinct architecture loses its identity. Lived in some Georgian terraced housing once that used to house law firms surrounding a court, it's that kind of thing. Doesn't apply much to barns though, there's no way you can fit a UK barn into small resolution art without losing the nuance of it.

Not for you specifically but for anyone else who's under the impression that you need local models for AI /d/ content, that is incorrect.
Mitigating the in-built censorship of modern large models is quite possible and for many fetishes is your best choice as smaller locally-run models may have difficulty with the logic required from them (like understanding that a giantess can not easily fit through doors or remembering that a woman turned to stone can no longer speak)

Been slowly working on a Pokémon h-game via Pokémon essentials for fun. I'm using ai for art as even if I could do it the amount of art I'd need isn't really feasible to do by hand.
Female Pokémon are Monstergirls, and the Male Pokémon are futas. I've got a menu system all set up where you can view H-scenes with any Pokémon in your party (with different H-scenes for each gender), unlocking more as you increase it's happiness. You can also access the scenes for previous evolutions of a Pokémon as well.
Current snag is having to go through all of the default UI code to rescale it for a bigger resolution, as the default is 512x384 and all the UI is hardcoded.

Suggestion: If you're going for a Pokemon theme, instead of fighting the aesthetic and UI limitations, lean into them. Go for smaller, simpler, pixelated sprites. In addition to reenforcing the classic and familiar aesthetics it will also mean smaller file sizes, make players more forgiving of limitations in the art, and will do wonders for making your AI-generated art less uncanny and off-putting.
Ex. Traced over this with a 56x56 grid of 10x10 px squares. Took like 10 minutes once I had the grid figured out.

If you're determined to use AI, I would think it'd be pretty straightforward to add a final stage of that converts your images into appropriately pixelated sprite's. It would cover up shit like Staryu's second thumb, Tangela's missing finger, Magnemite's non-Euclidean magnet, etc. etc. etc. The best way to use AI in a workload is through iterative processes rather than trying to get it to do everything in one go.

The game started as a very simple and straightforward trauma/rape sim where your character tries to balance school, work, and daily goings on in a community full of perverts and bullies, with a handful of characters and fetish features for flavor, but it's bloated to something unrecognizable since then; we've got multiple character routes full of supernatural shit and ancient rituals and forest spirits and Eyes Wide Shut cults and parasitic pregnancy and centaurs and Innoxia levels of transformations and wardrobe customizations and it's becoming harder and harder to recognize the game for what it started as.
Three lessons every game dev should take to heart: (a) narrow and deep is always better than wide and shallow, (b) don't release your game until most of your core content is finished, and (c) ignore requests that go beyond your original scope.

NTA, but for me what makes it uniquely bad is actually the dev doing the classic mid-development ideological troon audience capture, and the game as a result being very schizo in whether it wants to be a dark dystopian forced fem game or a chungus self-actualization game. So they let you "choose" for a lot of stuff, but there is still tons of other stuff in the game that is tonally clashing or straight up awful.

NTA, but the issue is that "voluntary" path basically lets you choose to not engage with the game. The game's plot would have to be rewritten quite extensively to incorporate it, which it hasn't been. So you get disjointed narratives, and mechanically end up in a situation where the boss sighs that there's no one to go to a party with, while MC goes "Yeah, good luck with that. See you Thursday for the same exact conversation?"
What I greatly dislike about a lot of "open world" feminization/sissification games is that they're not actually sandboxes. They want you to find a specific NPC and progress that NPC's questline, regardless of how you feel about said NPC. In Secretary specifically, the only way to get the "dom" stat is to engage with the "data wizard" transsexual NPC at work, and then dom her once you're in the D&D game she runs. There was literally no other option last time I played. Or, hey, what if you don't like booze and bars, and decide that MC shouldn't go hang out at the bar with the male work-buddy after the first, forced time? You never unlock the gym. Or prostitution.
It's like playing the adventure games from the 90s all over again. Doesn't matter if you've thought up a solution that works to bypass a particular puzzle. The only thing that matters is the solution that the dev was thinking of, and whether you can guess it.
