How do you fix this?
Do i have to sand or scrap all the green and old paint before repainting? Apply any product that only magic paint men know?
I have already fixed the roof leak

Scrape it with a putty knife. Scuff it with a scotchbrite pad or wire brush to knock off any additional loose stuff and then brush on or spray on new paint.

Your ugly ass gonna be in there with it. You could rebuild that wall from scratch and cover it with gold leaf and it would still be a net negative.

you could throw almost anything opaque over it and it would eventually be fine, but ultimately this is a question of aesthetics and theres no way to give you real advice without knowing what the hell you want it to look like when completed, for example, if you where a weeb i'd tell you to throw shoji or a wall scroll or something over it. a western scotch and a cigar larper then a book shelf to go with your pool table and bathrobe rack. it all fucking depends asshole.

I want it to look plain clean white. I am going to remove the heater and the pipes, and just leave the electrical box, give it a good cleaning.
I am going to make it as nice as possible with what i have there, the question is about how would an expert go about repainting this stuff so you get a good (just plain clean white) durable result. I am sure there is some special knowledge pros have about it.

bleach the algae to kill it, then sand it, then put on an oil based primer like kilz or zissner coverstain, then paint it and you are done

I am making some progress. Turns out i can sand out most of that shit, the concrete underneath doesnt look that bad.
However i will look into these products you mentioned.

it's primer. it makes the paint stick better. it's also white so if you don't give a fuck you can just leave it like that.
