I have some neck vein distension and it originates from the neck up to the ears. I have objective tinnitus or ear sounds but only 20 times a minute-ish. I have fullness of the ears and dry eyes.
The most problematic symptom I have that is that I can't sit or stand upright for long without chest pain getting worse.
This is not a test I need some treatment right now...

My whole fucking class is on "ADHD" "meds". Fucking junkies. But here I am drinking copious cups of coffee and eating zyns, but IM THE ONE WITH THE PROBLEM?! Should I just get on the amphetamine train like everyone else? Anyone can get le heckin ADHD diagnosis these days. My fuck. I'm not sure what happened to the curriculum but even young grads are saying that our current workload is higher than what they had 5-10 years ago. The content isn't hard either, but my fuck the volume is h i g h. Even Anki-ing for up to 600qd is just keeping me at pace for content.

Why were you worried about getting good marks on the exam then? Med school is «minimal effort club» you dork. Just get by. Learning extra punishes you on exams. Go hard in Y3/4 and especially residency for the pedantic shit.
>>

>>16969109
Man. This American culture shit is crazy. Medical school isn't even that difficult. Actually, the masters/PhD people don't "get" medicine because they're trying to master irrelevant pathways in hyperfocused detail. I've met some simple people who have been crushing exams and the dorks have been failing them. You only need a 110-115IQ to do medicine lmao. These fucking hoops are insane.
I-i mean... Medicine is hard desu! Be glad you're joining the ranks of HIGH SOCIETY. You better be a good student, iron press your white coat and watch THE PITT™®©. I fucking hate this culture and gatekeeping, but at the same time the gatekeeping is necessary somewhat. I'd hate to have a wide-open lottery, or have a diluted perfession. I just wish people would be less interested in medicine. Most people shouldn't even be in university. THEY SHOULD BE LOCKED UP INSTEAD

Yeah, the widespread use of Adderall meds is wild. If someone is diagnosed with ADHD as an adult and they made it all to medical school, it's unlikely they actually have ADHD.

And as an aside, it's awful that so many medical students have a Type A personality; they will do whatever is necessary to increase their scores. It explains why a 250 Step 2 score is no longer impressive (it's the 47th percentile now).

Involuntary psychiatric treatment causes patients to distrust doctors and disengage with treatment:
>Three quarters of the youth reported negative impacts of IH [involuntary hospitalisation] on trust
>Hospitalization may... diminish trust in providers... detrimentally impact self-worth
https://pmc.ncbi.nlm.nih.gov/articles/PMC10105343/
Medicine is supposed to be evidence-based. So look at the fucking evidence and realise that psychiatric care needs to be better, instead of the current system, which harms the people that it claims to treat.

how do i tell what medical specialty i want to do? i'm stuck between family medicine since I can do a DPC practice and general surgery. Hi /med/, I'm looking for some advice. My father was just diagnosed with a large mass in his lung. He's scheduled to receive a biopsy to determine what sort of cancer it is specifically (e.g. lung vs something else that travelled) and begin treatment asap. I'm already aware that the biopsy and potentially NGS and IHC are going to be necessary to put together a treatment plan. In the meantime, I'm trying to find data on anything he should be doing in terms of diet and exercise (or anything else really) to help improve his chances since he's already started losing weight and muscle mass. There's the obvious stuff like fruits, vegetables, nuts, and lean protein, but I'm wondering if there's something else he should be doing. If anyone has gone through this or is aware of any resources or papers I'd love to read them and would really appreciate it.

yeah but how realistic is this as an attending? and is that in Canada or the US? I'm in the US.
I think if I can be a DPC FM doc and get one good day a week of real procedures where i actually cut and stitch then that would scratch my procedural itch enough. and as FM doc I don't want to do contraception or vasectomy or circumcisions so when I say procedures I mean like dermatologic stuff. or maybe i should go FM with obstetrics? is FM DPC with obstetrics privileges realistic? I'd love to do C sections but don't i need so many a year just to break even? and no ones even having kids anymore esp not an educated middle class DPC panel.

you ever feel like your brain is near max capacity of memory? that's kind of what i've felt like lately.
i'm like halfway into my residency right now and i feel like i'm approaching the maximum storage for my brain and it's harder to internalize new information. i used to be really quick at recalling stuff. numbers, names. i could remember all the details about a thing days or weeks later. now it feels like less details are recorded and able to be called back. it's like i'm at 80 or 90% of the functionality i had a year ago.
i've been trying to figure out what the fuck is going on. is it stress? i had a "difficult" septoplasty like 6 months ago and i was attributing these mental symptoms to a post-anesthesia thing but that was a while ago now. i even quit drinking and doing any drugs other than adderall back before the surgery.
am i getting old and this just happens in your late 20s? . i feel like my brain is damaged. do i just keep trucking and maybe it fixes itself? i'm at a loss here.

Make sure to show up fully confident about what your diagnosis is, the more obscure the disease the better. Have tabs of WebMD and Cleveland Clinic at the ready in case they voice any skepticism.
Around nurses, be confident and lecturing (they're nurses, they don't *really* know wgat they're talking about). Around doctors, second guess everything they do. If he recommends one test, question him on why and request alternatives. And nitpick him on treatment options and differential diagnoses. They love that shit.

No one can heal unless he understands what purpose sickness seems to serve. For then he understands as well its purpose has no meaning. Being causeless and without a meaningful intent of any kind, it cannot be at all. When this is seen, healing is automatic. It dispels this meaningless illusion by the same approach that carries all of them to truth, and merely leaves them there to disappear. (https://acim.org/acim/en/s/541#1:1-5 | W-136.1:1-5)
>>
>>
Sickness is a decision. It is not a thing that happens to you, quite unsought, which makes you weak and brings you suffering. It is a choice you make, a plan you lay, when for an instant truth arises in your own deluded mind, and all your world appears to totter and prepare to fall. Now are you sick, that truth may go away and threaten your establishments no more. (https://acim.org/acim/en/s/541#7:1-4 | W-136.7:1-4)

All the American pre-reqs aren't in my country. I never took a single American pre-req and I'm doing just fine without the pedantic hoop-to-jump-through shit. Thoughts on antidepressants, docs?
I bet a lot of patients who go on them could actually fix their lives without them. They need self-belief and perseverance. Instead of a negative attitude like "you can't succeed without pills, you need them".
Then when a patient is on antidepressants, it's hard to get off. Stopping the drug causes withdrawal and the patient's functioning declines, which some people interpret as evidence of an underlying "mental illness", when actually it's just withdrawal.
Do you agree?

I keep wondering what forensic med must be like in the first world. I don't mean big American cities full of gangbangers, I mean like a city in Norway or something. Do they really just do 1 autopsy a week and if ever there's a homicide the entire department gathers around and goes "Whoa holy heck is that a heckin single gunshoterino???" That person probably needs a massive amount of valium or halcion to make it through even a cleaning, especially if drug abuse was involved in that situation. They're a huge pain in the ass to deal with because even basic stuff like fillings has to be treated like an outpatient surgery. They probably should just get it all out under IV sedation and get dentures at this point desu, it's just delaying the inevitable (they WILL complain constantly about the dentures "feeling unnatural" afterwards)
>>
>>
>>16989870
The ADA had to create a new specialty (ASDA) for this phenomenon which is much more widespread than it was 20-30 years ago. I cap myself at 5-10mg of valium and if that doesn't cut it then they're getting referred to a dental school for mutlispecialty care. It's crazy that someone would be willing to pay 1200$ for IV sedation and a filling instead of just sucking it up, getting a therapist/psychiatrist, and paying 200$ for a filling instead

Okay, you have a pretty good sense of humor, so I'll help you. Do you have any criminal record? If so, medical schools will scope that shit out so fast and you'll be out of luck. And are you sure you have your addiction under control? Medical school is stressful and I don't want you getting addicted again.
>i have a 4.15 GPA after my first year back in school if that matters. i just can't shake the feeling that i'm too late
Assuming you're American, you're not late at all. A lot more people are taking gap years because of increasing competition. I entered medical school at 24.

Seconding. I started undergrad at 25 and graduated med school at 33. Still have a 30-35 year career ahead of me.

You wouldn't be the oldest at all. I started at mid 20s in a place where people usually start at 18.
Also any surgeons on here ? I'm split between a few and want to know how Ortho is vs Neuro vs plastics in west Europe

>i'm a neurosurgeon in America. Plastics has a changed a lot since the advent of the internet.
>Rapid development resulted in emergency plastics-a very interesting & science rich field.
>We're not talking lipo & butt jobs-we're talking gel sultures & crafts from organs-improvization on an emergency scale!

>in fact 20 years you might go into say, cardiothorassics as em but these days plastics is the most prestigious-it's either neuro or plastics (i'm also a pediatrician, but i have a phd in physics from cal tech)

>What i did in med school originally was peds but i was minor/emergency. i was finishing a phd in physics & i decided to go into neurosurgery but u have to complete neurology & psychiatric in America for the Hippocratic oath version we have here for that.

Do hospitals have proprietary files that only staff can view pertaining to particular patients? Not medical files, but like a record of any complaints or comments on behavior between medical staff that wouldn't be on a clinical chart. Things like "pattern of complaints of perverted behavior from female clinicians." or "combative about ordering tests", "doesn't wear underwear". Is there a way you can get a heads-up on if your patient is a sex offender or violent behavior patterns?

>have a partly broken tooth
>maybe a bit of it is exposed
>still a lot of tooth left
>dentist said no to filling and recommend extraction
>don't know about other types of advanced dentistry, would have to check
If I can't have some sort of dental operation, do you think I can just keep it as is? I kind of get the potential infection thing. A lot of people run around with broken teeth though. Do I really need an extraction? Advice?

Since this is /med/ and not /dent/ this is what you're getting
>pic related
That being said, toothtards are known to all to be deceivers. Get a 2nd opinion.
