What if the legends and tales of mythical human fantasy species are actually based on people with disseases?
I clearly see koksal baba as a real case of an IRL hobbit.

Most humanoid fantasy races are basically humans but better in important ways that essentially makes human lesser than the fantasy races.
If you go by the "humans are the jack of all trades" thing in your setting.

There's a rare genetic disease called Harlequin-type Icthyosis. It cases newborns with it to look like picrel - spoilered as it's rather freaky, but in short, large white keratinized scaled with thin reddened dermis between them. Details here: https://en.wikipedia.org/wiki/Harlequin-type_ichthyosis
I've been toying with creating a fantasy species based on the appearance and traits that come with it. Newborns who don't develop and then die from an infection due to the exposed dermis between the plates usually have lifelong problems retaining moisture, so my first impulse would be to make these people amphibious. The heat and cold intolerance makes me think they'd want a neutral temperature, so I'm thinking a Mediterranean climate, possibly an island in a mediterranean-inspired sea.
Could be neat

I mean… they are pretty much. Every idea has an influence. Dwarves were created after midgets for example. You can find some disease or genetic condition that can “create” most humanoid fantasy races. The rest are human/animal hybrids.

The idea may work if executed properly as all things in fantasy entertainment ideas can be good when executed properly.
The problem with that is that the source of inspiration is a very sad one and I think it would make players genuinely discomforted upon discovering the source of inspiration.
So maybe don't talk about the source of it if you really want to go through with this. I personally would go back to the drawing board as the source of inspiration will genuinely make players uncomfortable.
No baby deserves to suffer. Especially not that.
You warned me and I looked and I still wasn't ready.
That hurt to see. That hurt to see the babies born with it.
I hope a breakthrough happens to cure these people of this condition so they can live normally.
God bless those afflicted by that. Hopefully I don't have the gene to pass this condition on.
If I do I'll think about adopting instead.

Might be a good starting point for homebrewing the look of a monster.
Just make sure the setting is grimdark so your people around you partaking in the game will be somewhat 'braced for impact' on something as sad and disturbing as that.

They didn't used to survive infancy. There's a pair of sisters I think who made it to their teenage years, last I heard. They were blind because their eyelids scratched up their eyeballs too much. Among many other issues as you might imagine. I doubt they're still alive, but it's been years since I last heard about them.
