I wouldn't really classify it as a CYOA but it does have a lot of VN elements because it's predominantly a text based game. If you're familiar with SLG VNs (So like Sengoku Rance and shit) it mostly gets its DNA from that genre.
So there are some RPG elements (Raising stats, recruiting followers, etc.)
There are some strategy/planning elements both long and short term
And then there are VN decision/choices that heavily influence what comes next etc.
There's also a semi-roguelike element where when you fuck up and lose you get to unlock bonuses that make subsequent runs easier. Some of these actually add new characters that have their own plots and shit which makes stuff equal parts easier and harder (One example is the girl from the OP who's a dancer that's actually the MC's half sister)

It's definitely not R18 levels but it's above simply fading to black. Some scenes are more explicit than others.

Generally speaking yes but fucking people who don't reciprocate usually leads to them either fucking off, hating you and working to fuck you over for the rest of the run, killing themselves or some combination of all the above.
Some characters you can fuck for just doing their quests, other characters you need one of the Sultan Game cards. So for example if you go to the Sultan and give him one of the sex cards he'll let you fuck one of his concubines but if you already used all your sex cards earlier in the run you can't ever fuck them. Shit like that.
