I've played through Blood and its expansions many times but only found this power up once early in the first episode. Have any of you encountered it in other places?

By doing a simple word search (CTRL+F) on each level's page on the Blood Wiki, I've found that it at least shows up in E1M3, E1M5, E1M6, E3M1, E6M3, E6M6, and CP06.
Keep in mind that these are the only times the power-up is explicitly mentioned on the wiki pages. There could be other appearances that simply weren't noted on the wiki.
