1. Zoe's power interfered with Akira's power directly. Her ability to compress and skip time made Akira's predictions unreliable, because the events he foresaw happened earlier than they were supposed to
2. The disguise Ramus tricked him by appearing as Zoe. He foresaw countering her blow, and didn't look further in time than that, as he had no reason to believe he needed to spend time and focus to analyze a future where he already seemed to have won
3. Earlier in the battle, Lucas just straight up did something that was not in the playbook of time. No tricks or time manipulation, Akira saw a future and was wrong. Given Lucas's power name "Star Line", we can infer the true nature of his ability: he literally rewrites people's fates.
