As far as I understand, fossilization can only happen in very specific conditions, hence certain places having shitloads more fossils than others; as a result, only a small minority of bones and other tissues are able to be preserved as fossils.
That being the case, it's quite plausible that only part of the specimen was kept in the right conditions.

no on both counts
fossilization means being incorporated into the crust of the earth for long periods of time
unaltered dinosaur bone is relatively common. Any paleontologist or bone hunter has seen it. Most museums have some. Some of it has been published.
