What exactly is insanity? Where is the boundary between sanity and insanity? And what causes someone's descent into insanity?

Whatever the contemporary society considers to be a mindset that is sufficently detrimental to its interest. Note that the interest of the individual is entirely optional for the diagnosis of a disorder, unlike all other, legitimate areas of medicine - you could have the textbook antisocial personality disorder psycho with little to no empathy that is nevertheless entirely satisfied with being a selfish dick and benefits from it, but it's still a disorder because it's detrimental to an arbitrary group of people other than the psycho. You can't escape the subjective social, cultural and philosophical biases in psychiatry, it's designed to fundamentally to prioritize the interest of a group over the interest of an individual, which is why it's a biased joke of a "science".
This is well illustrated by homesexuality - it was considered to be a disorder at one point due to culture, the zeitgeist changed, psychiatry followed.
