if you don't mind incomplete and 2dcg, I recently tried out Her Last Piece and while there isn't too much there, the LI is fully voiced. It was pretty hot, but you could do a single playthrough in a single sitting. Two paths have enough difference that I did two playthroughs and enjoyed myself on the second time around. Story is you're the last surviving member of your party sent to slay the evil queen but instead of killing you, she makes you her servant. You can then be lovey dovey and follow the romance path or sexy smexy man and follow the yandere route.
Race of Life's ex-wife path is kinda like that but also she can fix you. Its really well done and strikes that perfect feeling of reuniting with a past love and trying to figure what went wrong and working towards making it better.
Finally my top AVN all time has multiple women that its your job to fix in Pale Carnations. Of the 5 LI choices, 3 of them need fixin, one needs breakin, and the last one doesn't want to be fixed and doesn't want you fixed either as you struggle to find yourselves in a fucked up secret world together acting as each others' anchor to normalcy.

It's just the middle door between Sage and Quinn's room. You can check it a few times throughout the game during freeroams. ep1 when you steal the panties (no need for key, it's unlocked for some reason), ep 4 if you stay with Sage (key is on shelf behind Heather), and then ep7 during the HOTs party (key is in Sarah/Mel's room). All you get out of it is $ and renders, but the stuff inside changes. I think there's a mannequin in there as well during ep1 and ep4, and a sports bag that shows up in ep7 with the drill + duck tape, but I might be misremembering. I've always thought it was weird that one of the HOTs would need a drill, and it shows up ep7, which is the episode after someone runs by your window at night (when you're sleeping on the mattress, if you pause for a sec in the last render or two, there's a shadow that runs past the lights outside).

Sisterly Lust did go for the easy route of staging the same images with a different model substituted into the same scene with mild dialogue edits rather than full alternate routes, but I think that's fine for devs that don't want to do the full alt route approach. I like dedicated scenes, but it's not a must to be satisfying. That said, there's plenty of games that do have full alt routes, some of which barely anyone will play (granted usually due to a bad choice chain that makes it hard to line it up) and are only implemented for the sake of completion, and those very rarely take the opportunity to have any significant preggo route. I don't think they're better, but what happens is that games made in DAZ usually started many years ago and used some technologies that are obsolete by today's standards.
For example, one thing I've always hated about DAZ Studio is the facial expressions, they're awful. But DAZ has added support for FACS (Facial Action Coding System), which is compatible with Genesis 8.1 (2021) models and later. So every game that came before 2021 will use those unmistakable DAZ expressions that are ugly as hell, while games after 2021 will have a huge advantage, in my opinion.

It's still difficult to make decent expressions, but at least now it's possible.
For example, look at the facial expression in pic related. In my opinion, it's a very genuine and natural expression, with the smile "rising" from the mouth to the eyes, and this simple thing was practically impossible in older DAZ models.
