Did all the Targaryen kings knew about it or were there some who never heard it ? What would the chain king by king ? First Aegon the Conqueror (and his sisters) then Aenys. Did Maegor know ? Jaehaerys clearly knew and told Viserys whom we know told Rhaenyra but not Aegon II. In the show Rhaenyra told Jace but did she also tell Aegon III ? If he knew that would explain why while he hated dragons, he was willing to try to get them back. And then ? His sons (Daeron and Baelor) don't sound like they knew. And would have Aegon III told his brother ? Aegon IV certainly don't look like he was aware he was endowed with a world saving mission. Same question for all the kings after him

Aegon IV may have told Daemon Blackfyre if he really thought he was his heir. But the Blackfyres don't really seem to know. Unless fatgon never knew. Daeron II may have know hence why he tried to unite Dorne with the realm. And he may have told Baelor Breakspear. But did Aerys I knew ? It is clear that Bloodraven knew all these things all along even though he was never an heir but Bittersteel don't appear to have been very concerned, we know he distrusted the dreams of Daemon II, why would he trust those of a king who died almost 200 years ago ? It is implied that the reason why Aerys I was so engrossed in books and scrolls is because he knew something about the prophecy. But then is it his father or Bloodraven who told him ? And Maekar ? Did he knew ? From his father or his brother or from Bloodraven ? It would be fully in character for Egg to have known maybe it was another reason why he tried to hatch dragons. We know there were talks at his court about the Prince that was Promised. Jaehaerys II knew and so did Aerys II because he married his children to have the Prince been born from their line. But Aerys clearly wasn't concerned with the prophecy. And of course Rhaegar belevied himself to be the chosen one. But it's curious at this point in time, late in the Targ dynasty, it seems it wasn't just the king and his heir who knew about Aegon's dream but the entire royal family as well as important members of the court
I understand why GRRM and Condal thought it important to include that bit but I don't think they thought enough about the difficulties it creates for the storyline

The prophecy sounds like something that gurrm came up with. But it's clearly a later invention and hard to retcon in. Almost every single Targaryan succession would have broken a king-to-heir designation.
And I doubt that what we heard in the show is what gurrm came up with. Because what's in the show would be perfect Targaryan propaganda and should best be promoted widely throughout the realm to legitimize the dynasty. It's not bad writing. The whore was Dondarrion's main squeeze for most of the early episodes. She's in his tent in episode 1, outside of it too waiting for him to wake up. So they can continue his mission of populating Westeros with more redheads.
Then once Raymun became a famous knight she swoops in, and convinces the poor fool with 14th century understanding of female anatomy that she's pregnant with him. Now who could she have been fucking recently to get her pregnant? Hmm it's a mystery. The show even helpfully tells you that it's most likely A BOY, hinting at the funny but awkward truth that Raymun's heir will most likely be not of his blood. Duncan even cringes visibly at this on-camera, and the whore pretends not to know him and subtly communicates to him to keep his mouth shut. You might as well have had Dunk turn to the camera and say "seven hells, she's cuckoldin' i'm innit?"
Everyone sane understood that this is a wife's son situation, and despite so many things going well for Raymun there's sometimes weird little quirks to life where a well-meaning idiot can stay happy as long as he stays ignorant. But for some reason there is a massive amount of cope online where people invent complete headcanons about why the opposite is true, where she's not pregnant or Raymun is just playing along, and desperately trying to force those headcanons upon everyone else. It is meant to be a strange decision that shows that perhaps Maekar is a stranger man than initial impressions would suggest. He's a stern asshole initially but after the Trial he is pensive and melancholic after killing his brother, thinking hard about his family and everything that went wrong. He recognizes, just like the reader is meant to recognize, that his two sons who were raised as Targ princes became bad seeds and a large part of it had to do with the fact that they were not exposed to the seven kingdoms, did not experience the dangers of the world, did not sleep in ditches and eat hard salt beef. Comfort and courtly autism damaged them, possibly.
Thinking of this, of his dead brother and his unlikely allegiance for the hedge knight, Maekar decides what the hell and makes a strangle gamble with his youngest son. Maybe the dangers of the road are an acceptable risk if the alternative is to Egg just definitely turn into a cunt like his older prince brothers did.
It's supposed to be an unusual fairy-tale decision that feels like a gamble. And Egg is ultimately inconsequential in the big picture, Maekar in the book isn't all that warm of a father and already has a bunch of kids, Egg is like 30th in line to the throne and there are too many Targs. Besides, the arrangement wasn't meant to last forever - Dunk was only supposed to take on Egg as a squire for a couple of years, with them checking in routinely and having contingency plans.

So there's some 'magic' or just biology that is associated with (powerful) families directly, and it coincides strongly with the legal system around marriage and names presumably because the latter is based on the former (in some long-forgotten history), or because somehow, people treating a family as such causes it to manifest that way physically.

In theory the scene does make sense. It aims to show that Aegon is too blinded by rage and grief to make sensible decisions at a critical point, which rears its hand again later. It's also meant to show that Otto is incredibly frustrated by the fact that despite decades of plotting, the monarch he desired to crown turns out to not give a shit about what Otto thinks. This is a huge blow to the old man.
However they executed it in an utterly retarded way and the scene makes no sense in so many ways. #1 Otto knows how to react to shit situations and this seems like a bizarre overreaction. #2 Otto going "your father was right about you" is nonsense because in the show Vissy T never utters a single word about Aegon so we have no idea what Otto is even saying or why we should care. #3 Otto's outburst is painted as if he's 100% in the right even though he isn't. #4 The smallfolk would really not give this much of a shit. Baelor went into the fight with the knowledge that all 7 of his foes are very unlikely to actually hurt him (barring freak accidents) and he took advantage of his own royal position and the Kingsguard oaths to make a mockery of the proceedings. He also didn't join the trial just out of honor, but because it was a political move to ensure house Targaryen's public image isn't ruined by the outcome of the trial.
The others went into the fight knowing all 7 opponents are out to kill them and risked their life for nothing other than honor or loyalty. Lyonel is right to be pissed at Dunk for being sad about Baelor but not acknowledging the deaths and sacrifices of others who risked way more.
