Both the shotgun and sandwich are good choices, but unless your team has a competent medic (i.e. he's actually healing the whole team and not just his friend, and he knows to buff powerclasses with overheal), you should be running the sandwich or banana. The only exception is if you're running the brass beast, in which case you should /always/ take a shotgun. Also, if you're not going to be taking a lunchbox item, take the GRU instead of the FoS so you can actually run around to healthkits, instead of trying to waddle to them impotently with the FoS, only to have them stolen by your teammates.

I am steadily accruing hours on heavy so that he’s no longer my least played class. What’s the best loadout for when you’re playing offensive and your team does not have a medic? Get the damn shotgun. Nothing else matters. If medic gets blown to pieces by a bombarding soldier it's his problem. You can flinch snipers from far, shoot on top of obstacles for spy check and remove stickies from doorways without needing to move at inch-by-inch pace.

Get the Sandwich if medic is bad at staying alive, get Steak if medic can dodge shit for long to deploy more than one uber in one life. The idea is eating Steak with FoS so you can make your escape when medic died. Longer cooldown is not really a downside because a good spy will hunt you while you eat that thing for 4 seconds anyway.

Get the banana. You can refill it easily if you divert away to flanking routes and shorter cooldown allow you to share it often. You can extend away from the nest but going too far is a risk. Ask engineer to move forward with the team or only attempt to push when you have more than one medic at your tail.

Get the dalokoh's bar. You can eat and still share it with short cooldown. Extra health and small overheal are actually pretty useful to give you a last push. That will allow you to incorporate melee attacks in between minigun volleys because you can endure more hits. You can wreck unattended alone sentries as well.

Because the game is barely optimized. It was coded for 2007 hardware and barely makes use of the GPU, because it was designed for when running off a CPU instead of a dedicated GPU was more common.
Then you start factoring in cosmetics. Any cosmetic that attaches to the body is simply modeled over and covers the existing geometry. Nothing underneath is culled out. The game is supposed to have body groups to solve that problem but there are very few actual body groups and Valve is content to keep it that way. That means your game is rendering Heavy's body twice every time you put a jacket on and now consider that you have 24 - 32 players all wearing 3 cosmetics plus a badge each. Now consider that those players could be wearing unusuals that are spamming particle effects too.

Then you get into the fact that maps are now made by the community and not everybody knows how to optimize those, either. I could go on.

144 fps and 144 hz are very different things

a 144 refresh rate is a steady refresh rate where you get a new frame every 1/144 of a second

144 fps means you are getting 144 fps each second, but these are not stable frames. For all you know you could be getting one frame for the first half of a second and 287 frames for the remainder of that second, and that would still be 144 fps

this is why you actually get smoother visuals at higher fps even if you already have a higher nominal fps than your monitor refresh rate

TLDR: Refresh rate is an even, steady spread. Frame generation (FPS) is not steady, your GPU just spits out frames in an uneven way, which will not match your refresh rate's timing.
