First you want to connect two cylinders with one cylinder being hexagonal. When you're connecting two things it really helps if they have the same number of vertices so the round cylinder has 12 vertices and the hexagonal also has 12. Then subdivision, 3 segment bevels to retain the edges and raise the middle of the hexagons. Tell me if that makes sense.
