Find a stranger face to face whom you want to talk to in any social setting by saying your greeting and asking if they have time.
Introduce yourself and learn theirs in their hobby or interest. If their personality is something you like then ask them if you can be friends with them and ask for their contact.
Even if their interest is not gaming, atleast they have knowledge in other area that can request help with especially farming, fishing, electritian, doctor such and such, what matters is you and they enjoy time being together.
Once your trust and theirs are deep and both are honest its called friendship.
Talk when you feel like it, don't force it.
Remember to say "no" when you don't feel like it.
Avoid friends with opposite gender since its risky so keep some boundaries.

Friendships are usually made when you have more than one common interest. The only friends I've keep after meeting them on a game are the friends who I've been able to play other games with too. If you have a friend on your MMO already, you can ask them what other games they like. Or if you see them on another game you play, you can suggest playing that together. It won't always work out. Some guys I like playing one thing with but I find them annoying on other games.

If you think the problem is yourself, you might need some self-reflection and change things you think are problematic. Do you usually make negative comments or talk about real world problems? People when playing a game usually want to keep it positive and ignore the real world. The friendship is for fun, keep your baggage out of it.

Well, that's good that you don't dump your shit on people. I don't think you're doing anything wrong outside of not having more than one game in common with people. That was the difference for me between making a friend and not making a friend. Have multiple things in common that you can do together and/or introduce each other to new interests.
